Formulary Information

VISTA uses a formulary which is a specific list of prescription medications, both brand and generic. Prescription drugs that are not on the VISTA formulary drug list are considered "Non-Formulary" and may require a higher copayment. Members that are enrolled in a VISTA commercial benefit plan, which includes employer group coverage plans as well as the Individual product, can use one of the VISTA Commercial Formulary links below to determine whether a specific prescription drug is on VISTA's formulary.
