Transition of Care

VISTA takes pride in ensuring the delivery of excellent service to our members. To assist you and provide a smooth transition to our plan, our Medical Case Management department needs information regarding your medical needs if you are in the midst of active treatment, for example: chemotherapy, pregnancy, scheduled surgical procedures or you are currently utilizing durable medical equipment.
